City of Dryden Contractor Documentation Upload Portal

Thank you for supplying the necessary documents for the work you have been contracted to perform for the City of Dryden. By uploading completed documents, you are helping to make our workplace safe. 

The following must be uploaded for all projects five business days before the project start date:

• Contractor Health and Safety Responsibility Agreement
• Sign-off Form for the Confirmation of Receipt and Understanding of HR-HS-55 Infection adn Disease Control Policy Ontario
• Current Workplace Safety and Insurance Board (WSIB) clearance certificate 
• Proof of current liability insurance coverage with no less than $2 million per occurrence of public and property liability 
• Proof of equipment certification where required (trech boxes, boom cranes, etc.)
• Utilize qualified workers with proof of appropriate staff training where required (Transportation of Dangerous Goods card, MOL Safety Awareness training, Workplace Hazardous Materials Information System training, Working at Heights, etc.)
• Copy of the Contractor's Health and Safety Policy and work procedures
• Particulars of any convictions or orders imposed under Health and Safety or Environmental Legislation
